by Dr Chan Joon Yee This is an example of a "simple", straightforward case as far as implant treatment went. The patient started off with a very loose central incisor which was just hanging by the gum thanks to advanced gum disease. The tooth was extracted and a denture was placed. However, the patient's smile line was slanted. We referred her to Dr Tan at Aline to correct the slanted smile line. While she was undergoing her orthodontic treatment, a one-stage surgery was performed with the insertion of a 4.5mm wide Bicon implant.
![]() Bicon implant with healing abutment in place When her orthodontic treatment was completed, we went in to take impressions and fit the patient with a definitive abutment. As expected, adjustments to the abutment were necessary to ensure better aesthetics.
![]() Definitive abutment adjusted in the patient's mouth For all cases with strong aesthetic concerns, we would prefer to take two levels of impressions, one at implant level and one at abutment level. This means an extra visit for the patient which may prove well worth it.
![]() A PFM fitted The final PFM crown, made by ceramist Mr Alvin Lim, looks even better and more "natural" than the patient's natural incisor. Further improvement can be made to the gum contour with periodontal plastic surgery.
