Immediate Post-Extraction Implants
by Dr Chan Joon Yee

Simply put, it means extract and insert immediately. This is something we prefer not to do unless the patient is willing to take a little risk. Nobody wants to undergo 2 surgeries, one to remove the broken tooth and one to place the implants. If someone has a choice, he might want to have an implant inserted immediately after the extraction. Can it be done?

Well, yes, but it's a challenging operation. Why? In healed bone where the tooth has been extracted for 2 months or more, there is practically no sign that a tooth was ever there. The dentist can drill into the ridge and the drills will generate a precise hole to receive the implant.

Now imagine you've already got a hole in the wall you'd like to drill. What are the chances that the hole in the wall is not the ideal shape, size and position that you want? Very high, because you didn't place that hole there. Similarly, a tooth socket in the bone after extraction is very unlikely to hold an implant snugly. As the surgeon does not have good control over the size and shape of the hole in the bone, the implant position may not be ideal. Thus, the immediate placement of implants into fresh extraction sockets is a challenging procedure.

We normally do not recommend this. If possible, wait for 2 months for the socket to heal so that a more ideal preparation can be made.

As usual, there is a system which claims that it can solve the problem for the surgeon. It's a Korean implant called MegaGen Rescue®. This system employs a different type of drills which allow the surgeon to produce a relatively snug fit for the immediately inserted implant.
